Family Practice/Primary Care Physician Assistant Job in Granada Hills, CA

Our client, an established and growth-oriented provider organization, needs an experienced Physician Assistant to provide post-discharge clinical care for a select patient population.

We are seeking a Physician Assistant who is innovative and passionate about identifying ways to improve patient care. As a key member of the Priority Care Team, you will manage patients as they transition from inpatient care to a community or home-based setting.

Collaborating with the PCPs, Population Health, and Case Management, you will provide high-quality, transitional care for more complex patients.

RESPONSIBILITIES:

Provision of patient care in outpatient clinic and/or home settings.
Provides high quality care to patients as they transition from hospital to home.
As an integral member of the Care Coordination Team, works closely with PCPs, Priority Care Physicians, Social Workers, Case Managers, and Pharmacists
Attends interdisciplinary meetings to ensure optimal coordination of care.

SCHEDULE:

Full Time, Monday Friday Schedule
No nights or weekends
